Important information regarding athletics

All students who participate in interscholastic sports are required to have an annual sports physical. This health assessment is the responsibility of the student’s parent or guardian. The exam will include a hemoglobin, which consists of a finger prick, a chronic disease assessment, a height, weight, and blood pressure check. Screenings for hearing, vision, speech, gross dental, and posture are also done. Postural screening is done to detect any abnormalities in the curvature of the spine. Physicals must be done on an annual basis. A two week grace period is given beyond the one year period, if you are unable to make an appointment within the one year period. No students will be allowed to participate in practices or games until a current physical is on file meeting Connecticut state guidelines.

Please fill out the front of the Sports Physical Form (State of Connecticut Health Assessment Record) and the physician will complete the back. Please return the Sports Physical Formas soon as it is completed by your child’s physician to keep on file in the Cumulative Health Record.
